Scope of Service Notice
We understand that health insurance can be complex, but our goal is to simplify the process for you. Please keep in mind that your insurance contract is directly with the carrier. This notice documents how Health Insurance Advisor, LLC fits your needs, and is dedicated to assisting you to the best of our ability. We believe that prompt communication and a clear understanding of responsibilities will save time, minimize expenses, and ensure compliance. Please read this thoroughly.
Description of Services – (Scope)
Health Insurance Advisor, LLC is a Health Insurance Broker. In brief, the company and/or staff will provide the following services (collectively, the “Services”). Onboarding, Enrollment, Maintenance, Renewal, and Termination services. For a full outline of services, see Appendix A.
Term
A consumer’s consent for Health Insurance Advisor, LLC to provide continued services will remain in effect, indefinitely, until terminated by either party. To qualify as consent, all service requests shall be delivered in writing, and sent mail or email to the respective parties' designated representatives.
Methods of Contact
In a medical emergency, contact 911. For coverage questions for a service or medication, call the number on the back of your card. Health Insurance Advisors, LLC operates between 8 am CST – 4:30 pm CST Monday – Friday, and may have extended hours at their discretion. Outside of standard operating hours, please contact the number on the back of your card. Otherwise, feel free to call us at 515-225-9994, and/or respond to emails with general questions.
Disclosure of Business Structure
Health Insurance Advisor, LLC, is domiciled at 5870 Merle Hay Rd, Suite A, Johnston, IA 50131, and services clients in where they hold licenses.
Rescission
You may rescind your consent by providing written notice of rescission. The request shall be effective 60 days from the date of receipt of such notice. The request shall be delivered in writing and sent via certified mail or emailed to Jvanahn@health-insadvisor.com.
