I have reviewed my Annual Notice of Change and confirming that I am happy with my current plans to stay as is for the next year. 

I would like to Review Without an Appointment 

I have reviewed my Annual Notice of Change, or, I have medication changes I would like to review but do not need to schedule an appointment we can communicate via phone or email about possible changes. 

Due to schedules being full, we are not accepting any more lists for review for the Annual Election Period 2025. We apologize for any inconvenience.
